Fair warning: almost all these fics to some extent or another feature hurt!Sam. If you don't appreciate battering the fuck out of Sammy, these fics might not be your favorite compared to those which batter the fuck out of Dean (but a lot of authors listed here are equal opportunity whumpers & I've just selected their Sam-focused fics)

Gen Case Fics
In Reverse by sodakey. Summary: After Faith, a job has the boys looking for missing hikers in Wyoming. While Sam worries it’s connected to what happened to Dean ten years ago, Dean wonders if Sam would be better off back in the world of normal. // Saved the best for last here. This story is unbelievably well-written, insightful, and believable.
Foreseen by P. L. Wynter. Summary: In 1967, tragedy struck Point Pleasant, West Virginia. They were given their warnings. No one listened. Now, the warnings are back. Rated M for language. // The author did her research on the Mothman myth & legend for this. Again, this is a fantastic thriller/drama. A lot of creepy suspense in this one.
Hozho by LadyVic Summary: Misunderstandings and insecurities throw the brothers off balance and land them in the path of a wildfire as they face a dangerous foe and the line between hunter and hunted blurs. // The author did her research on Native American myth & lore for this fic and threw the boys into an extremely compelling thriller/drama.
Until It Sleeps by ucat42. Summary: Every 22 years something crawls from the bowels of the Earth to wreak havoc and death. This time, it is back early, and it's escalating. Bobby joins the Winchesters in this hunt. Hurt!Dean, angst, Hurt!Sam. AU Set season 6, roughly around Ep 12
Obsession by A-blackwinged-bird. Summary: Sam faces his fears though he'd rather not. // OCD Clown serial killer. Need I say more? IIRC, this fic is heavier on hurt!Sam than comforted!Sam, but it's still pretty good (great villain)
Red by big-pink. Something evil is killing treeplanters in the forests of the Pacific Northwest, possibly the same predator that Dean narrowly escaped years before. How Grimm will things get before the brothers figure it out?

The Obeisance of Memory by rockpaperscissor. Summary: AU of season 4. COMPLETE. "Look, I've got no idea who your brother is, but uh, I think I might be him. Just, y'know, alive."
Unchangeable by HunterPeverell . Summary: Something happened to Sam. Something magical. When he woke up the day after Dean became a demon, it was to the year 2008 . . . the day Dean got out of Hell. How will Sam's attempts to fix this time work out? Who sent him? And how can this help Sam with his own demonic brother in their time?

A City Sorrow Built by Phreakycat. “In which Sam cries in his sleep a lot and Dean tries to fix it.”
From the Darkness by K Hanna Korossy. "He was terrified. This wasn’t what I’d signed up for.”
Illusions and Thump in the Night by Kyriebess. Respective summaries: "Sam gets attacked by a rare creature who is trying to scare him to death. Fortunately, there's a small but slightly awkward loophole in the creature's spell that may help the brothers defeat her" and "Under a lot of stress, the brothers argue, causing a monster to attack. // Basically if there had been a web series of short half-hour episodes to coincide with season 1 of SPN, these two stories would be featured.
Fishing for Mermaids by CeCe Away. Summary: Don't be dead. Don't be dead. Sam floated, legs angled downward by the weight of his heavy wet boots and jeans. // This fic is action-packed from start to finish and features a creature that SPN hasn't tackled before: mermaids! I literally couldn't put this fic down. I had to reread it again because I was zipping through it so fast the first time. Totally felt like an episode - only better.
None Goes His Way Alone by Phreakycat. Summary: Dean can call him Daryl Hannah all he wants, if he'll just get here already. He's starting to raisin. And the water creeps higher." Featuring Sam in peril, Dean to the rescue, and Awesome!Bobby.
Four Years And One Week By: Onari What can you say or do to help the most important person in your life when you have stopped knowing him and his life has come apart? A fic about the week in Standford right after Jess death. Dean POV.
Fellowship of the Nine By: emebalia Sam spends a weekend with some Stanford friends. But he's not the man they used to know anymore. Outsider POV. Season two. Sequel is Two Brothers: "I didn't expect to see Sam again. For sure I didn't expect him to die on my operation table." NOT a death!fic, sequel to "Fellowship of the Nine". Season 8.
Ambush by Rachelly. Summary: Dean and Sam run into serious trouble when hunters gang together and make Sam their prey, and Dean, his only hope. Sam angst, Dean angst, hurtSam, hurt Dean.. NOW 1595 reviews! Thanks guys!

Is Hard Gen Sam&Dean a Thing? Hard Gen = heavy intimacy that's really only technically platonic. Also, many of these are alternate universes (!!!)

My all-time favorite J2 fic is Tails by keep_waking_up. Summary: Jared couldn’t imagine the kitsunes had ever lived without the werefoxes to look down on and this was just another example of it. Jared Padalecki is a homicide detective for the werefox police, up until the kitsune police department decides to recruit him as their ‘Fox Consultant. There is nothing Jared wants less than having to work with the kitsunes, but he doesn’t have much of a choice. Of course, on the first day of his new job, one of the city supervisors is murdered and an official from the US government is flown in to handle it—one Jensen Ackles, an eight-tailed Heaven kitsune. Jared isn’t sure what’s going on, but he just wants to lay low and stay out of trouble. Unfortunately, fate—and Jensen—have other plans.

Incomplete but what will probably turn out to be in my top 5 of all-time favorite J2 fics: Iron Ridge Pack 'Verse by meus_venator (Prime is probably comparable - a brutal, intense read like most of her stories - but I think I'll end up preferring Iron Ridge Pack 'verse)
The Extraordinary Experiences of an Unusual Sidekick In the City by truelyesoteric. Summary: Jared Padalecki, Class Two Superhero, has lived his entire life in his small town. Another disappointing Superboyfriend is the kick that he needs to get out. He arrives in the City, expecting to be overwhelmed, instead it seems that he finds everything that he has always been looking for.
Wincest
Do they have to be complete? Whenever I think of the most intense, stays-with-you-for-years Wincest fic I've ever read (and Wincest's my one & only ship besides J2, I suppose), it's Freak Camp: A Monster by Any Other Name by Brosedshield, LaviniaLavender, whereupon. Summary: Our story takes several steps away from canon. Dean Winchester is an only child. Mary died November 2nd, 1983, but neither in a fire nor on the ceiling. Five-year-old Sam is deported to a concentration camp for monsters. But the core remains the same. This is a love story.
Blue Skies From Rain series by lovesrain44. Summary: The morning after Sam rescues Dean from the djinn, Sam and Dean go back to the warehouse to take care of the bodies of both the victims and of the djinn. But instead of what should be a simple clean-up job, Sam and Dean are sucked into a nightmare world brought about by the djinn's last dying act of revenge. (Takes place directly after What Is and What Should Never Be.) What do you do when you wake up in a mental institution and you think your brother is dead?
Courting Death by theproblematique. Summary: Sam Winchester lived the first six months of his life in a happy family; the next twelve years as John Winchester's only son, and the last decade as an orphan. He's supposed to die at twenty-two trying to save the woman he loves from a fire, because he doesn't have a brother to pull him back. But the night Sam meets his Reaper he discovers that Death is overly fond of pop-culture references, too beautiful to be real, and reluctant to kill him.
Catching Hell by ADeedWithoutAName. Summary: Sam Winchester is, perhaps, the greatest asset to the hunting community. His research, advice, and insight has reached countless hunters and solved innumerable mysteries. His life is solitary, but useful. That changes, however, with the delivery of a captured Knight of Hell who calls himself Dean.
A Fire Ever Burning by BewareTheIdes15. Summary: Sam and Dean have always been oddities among the dragonriders; the youngest man to Impress a Bronze dragon in a hundred years and a boy who turned his back on the riders to become a dragon healer instead. But when fate steps in and chooses a far different path for Sam, the brothers and all the Weyrs of Pern will have to come to terms with much greater changes in their traditions.
Sure Got A Dirty Mouth by Justine DeLarge. Summary: This is NOT just a smutty story. This explains why Wincest is beautiful, loving, and right. Ever wonder how Dean started talking dirty? The genesis is in the way Sam and Dean allow their feelings for each other to become physical. Sam is nearly 17, Dean is nearly 21.This story has dirty talk, all the feels you can handle, infinite love, well-developed original characters, and a rich plot that develops into a nail-biting narrative, with the best kind of hurt/comfort. SPOILERS for seasons 1-6.
The Promise by Gillian Middleton: Kidnapped and imprisoned, Sam & Dean only have each other and their inner resources to get them through. warning - wincest. // My wincest gateway fic (ahhh! lol)
